The Role

The Pathway to Good Health program provides comprehensive health screening, assessment, care planning and referral for Victorian children and young people aged 0–17 years who are entering or re-entering statutory care, including those in kinship, foster and residential care settings.

In the Mallee Child Protection Area, the program is delivered by Sunraysia Community Health Services from the Family & Child Hub. As the designated health provider for the Mallee area, SCHS works in close partnership with local Child Protection services and carers to ensure health services are coordinated and delivered in the most accessible way possible, based on where the child lives.

Health Navigators perform a critical function in the PTGH program to ensure all eligible children receive initial screening upon entry to the program and are pivotal in supporting the connection with Child Protection practitioners to build strong and respectful partnerships

The Pathway to Good Health (PTGH) Navigator delivers health screening, assessment, planning and referral to children and young people aged 0-17 entering or re-entering statutory care in the Mallee. This includes children and young people in kinship, foster and residential care settings.

Key Selection Criteria

Mandatory

  • Current registration with the Nursing and Midwifery Board of Australia (AHPRA) as a Registered Nurse.
  • Minimum three years’ nursing experience, preferably in a community-based setting.
  • Proven knowledge of child development and attachment theory, with practical experience in paediatric triage, biopsychosocial assessment, care planning, and advocacy.
  • Applied understanding of the social determinants of health.
  • Current Victorian Working with Children Check (Employee), or the ability to obtain and maintain one prior to commencement.

Desirable

  • Experience supporting diverse, complex, or vulnerable families using trauma-informed, relationship-based approaches.
  • Knowledge of health, social, and education systems and referral pathways, including the NDIS.
  • Highly developed engagement, communication, and capacity-building skills.
