Job description

Enrolled Nurse - Home Care 

An opportunity is available for an Enrolled Nurse to step into a Clinical Coordinator role supporting residents within a retirement living and community care environment. The position focuses on coordinating integrated care services, ensuring residents receive the appropriate support to maintain independence, wellbeing and quality of life.

This role works closely with residents, families, care teams and external providers to coordinate clinical and community-based services, provide medical assistance and ensure compliance with aged care standards and funding programs.

Key Responsibilities

  • Provide holistic clinical case management for residents, promoting independence and enablement
  • Conduct pre-entry and ongoing assessments to determine care needs and appropriate support services
  • Develop, implement and regularly review comprehensive care plans
  • Liaise with residents, families and external providers to ensure continuity of care
  • Facilitate case conferences and risk assessments where required
  • Monitor and guide Care Service Workers to ensure services are delivered to the required clinical standards
  • Maintain accurate documentation and ensure compliance with aged care quality standards and regulatory requirements
  • Support resident wellbeing by encouraging participation in meaningful activities and social supports
  • Contribute to quality systems that support compliance with Aged Care Quality and Safety Commission requirements

About You

  • Enrolled Nurse with current AHPRA registration
  • Minimum 1 years experience in a similar clinical or community care role
  • Understanding of Support at Home and relevant funding models
  • Strong knowledge of community aged care programs
  • Experience coordinating care within retirement living, community or residential aged care settings
  • Ability to build strong relationships with residents, families and multidisciplinary stakeholders
  • Strong documentation and administrative capability
  • Intermediate Microsoft Office skills

Qualifications and Requirements

  • AHPRA Registration 
  • National Criminal History Check
  • Driving License
