Disability Support Worker

About the role
We are seeking a dedicated and experienced Disability Support Worker to provide person-centred support to older people and people with disability. The role supports clients to maintain independence, wellbeing, safety and meaningful participation in their home and community, in line with individual support plans and organisational requirements.
Key responsibilities
- Provide personal care and daily living assistance, including support with mobility, hygiene, meals, medication prompts and household tasks, in accordance with each client's support plan.
- Support clients to develop and maintain independence, life skills, social connections and participation in community, recreational and appointment-based activities.
- Observe clients' physical, emotional and social wellbeing; identify changes or risks and promptly report concerns to the relevant supervisor or health professional.
- Implement individual support plans and follow instructions from health professionals, family members and the care team where appropriate.
- Provide companionship, emotional support and respectful communication while maintaining client dignity, privacy, choice and cultural safety.
- Assist clients to access services, attend appointments and use transport safely, where required.
- Maintain accurate case notes, incident reports, progress records and other required documentation.
- Follow workplace health and safety, infection control, safeguarding, privacy and duty of care requirements.
Selection criteria
- Certificate III or Certificate IV in Individual Support, Ageing, Disability, Community Services or a closely related field.
- At least two years of relevant full-time experience supporting older people or people with disability in a residential, home-care, community or disability-support setting.
- Demonstrated ability to provide personal care, support daily living activities and implement individual support plans.
- Sound understanding of client dignity, choice, privacy, duty of care, safeguarding and professional boundaries.
- Clear written and verbal communication skills, including the ability to complete progress notes and incident documentation.
- Ability to work independently and cooperatively with clients, families, coordinators and multidisciplinary teams.
- Strong organisational skills, reliability and a respectful, client-focused approach.
Mandatory requirements
- Current NDIS Worker Screening Check or ability to obtain one before commencement.
- Current Working with Children Check, where required.
- Current National Police Check.
- Current First Aid and CPR certification, or willingness to obtain before commencement.
- Valid driver licence and access to a reliable motor vehicle.
- Two professional references from recent employers.
Desirable attributes
- Experience working with NDIS participants or aged-care clients.
- Experience supporting clients with complex needs, dementia, mobility limitations or behavioural support requirements.
- Ability to work flexible hours, including weekends, where required.
