Community Worker

About the Role
We are looking for an experienced Community Worker to join our team in Mawson Lakes. In this role, you will help participants access suitable community activities, services and resources and build meaningful connections within their communities.
You will coordinate community connections and referrals, identify opportunities and barriers to participation, and work closely with community organisations, service providers and support coordinators to promote social connection, community participation and greater independence.
Key Responsibilities
- Work with participants to identify their community participation needs, interests and goals, and develop practical strategies to support their involvement in the community.
- Research and identify suitable community services, resources, programs, social groups and recreational activities based on participants’ needs and interests.
- Provide information and guidance on available community resources and connect participants with relevant organisations and service providers.
- Identify barriers to community participation and help participants find practical ways to access suitable activities, services and resources.
- Plan and coordinate social, recreational and community activities that encourage participation, social connection and greater independence.
- Support participants to build confidence, develop social connections and become more actively involved in their communities.
- Advocate for participants’ needs, choices and preferences when accessing community services and activities.
- Build positive working relationships with community organisations, service providers, support coordinators and other relevant stakeholders to coordinate referrals and service connections.
- Monitor the effectiveness of community activities, referrals and service connections and adjust where needed to improve participation outcomes.
- Contribute to the development and review of community activities and programs based on participants’ needs, interests and outcomes.
- Maintain accurate records of referrals, community activities, service connections and participant outcomes.
- Work in accordance with NDIS requirements, person-centred practice and Nurse Aid Australia’s policies and procedures.
