Spearhead Supports Pty Ltd is seeking compassionate, reliable and experienced Disability Support Workers to join our team in Perth, Western Australia.
We are recruiting for permanent full-time positions to provide person-centred support to people living with disability. Successful applicants will assist participants to maintain independence, achieve their individual goals and participate meaningfully in their home and community.
About the Role
As a Disability Support Worker, you will provide practical, personal and emotional support according to each participant's individual needs, support plans and goals.
Key Responsibilities
- Deliver person-centred disability support in accordance with individual support plans.
- Assist participants with personal care and activities of daily living.
- Support participants with mobility and safe transfers where required.
- Assist with meal preparation, household activities and daily routines.
- Provide medication assistance within the scope of the role and organisational procedures.
- Support community access, appointments, recreational activities and social participation.
- Assist participants with travel and transport where required.
- Promote independence, dignity, choice and social inclusion.
- Maintain accurate progress notes, incident reports and other required documentation.
- Identify and report changes in participant needs, risks or wellbeing.
- Communicate effectively with participants, families, colleagues and relevant health professionals.
- Comply with workplace health and safety, safeguarding and organisational policies and procedures.
Skills and Qualifications
- Certificate III or IV in Individual Support (Disability, Ageing/Aged Care or Community Services), or an equivalent relevant qualification.
- Minimum 1 year of relevant experience in disability, aged care or community services preferred.
- Current First Aid and CPR certification.
- Current National Police Clearance.
- Current NDIS Worker Screening Check, where required.
- Working with Children Check, where required for the duties.
- Valid Australian driver's licence.
- Strong interpersonal and communication skills.
- Good documentation and record-keeping skills.
- A caring, respectful and person-centred approach.
- Ability to work independently and effectively as part of a team.
