Your next employer:
Join a Home Care provider that supports older people to remain independent in their own homes. The team works closely with clients, families, support workers and health professionals to make sure care remains appropriate as needs change.
Are you a Registered Nurse who is calm, experienced and comfortable taking ownership of client care? Someone who can build rapport, make sound decisions and be a steady point of contact for both clients and the team? The stars might be aligning!
The opportunity:
You’ll take the lead on care for clients with more complex home care needs, making sure the right services are in place and adjusting them as circumstances change. It’s a role that draws on your nursing judgement, but most of your time will be spent managing care rather than delivering clinical visits. It’s a role for someone who enjoys the relationship side of Home Care as much as the clinical side and who can bring a clear head when plans need to be adjusted.
- Meeting new clients, completing assessments and helping them get started with services.
- Reviewing care plans regularly and whenever a client’s needs change.
- Keeping an eye on package budgets and making sure services match each client’s priorities.
- Visiting clients at home and staying in touch with their families.
- Working with allied health professionals and other providers to coordinate care.
- Giving support workers clear guidance when a client’s needs or care arrangements change.
- Providing clinical care when needed, including wound care and health assessments.
- Keeping client records accurate and up to date.
What makes this great:
- Permanent, full-time position paying $50–$53 per hour, depending on experience.
- Work vehicle, fuel card and laptop provided.
- Monday to Friday hours, generally 9.00am – 5.00pm; other start and finish times can be discussed.
- A chance to follow clients’ care over time, rather than only seeing them for individual visits.
- A good balance of client contact, care coordination and occasional clinical nursing.
About you:
This is an ideal opportunity for an experienced Care Partner, Care Manager or Community Aged Care RN. You know how to assess a client’s needs, write a useful care plan and have an honest, reassuring conversation with a family when things change.
- AHPRA Registered Nurse
- Experience with client reviews, care management or home care packages.
- Confidence supporting workers and working with allied health and other providers.
- Sound clinical judgement and willingness to provide hands-on nursing care when needed.
- Clear documentation and confidence using electronic client records.
- A current driver’s licence and willingness to visit clients in their homes.
- Current First Aid and CPR certification, or willingness to obtain them.
