Join Salem Home Care team based at Toowoomba for a contract role until Nov 2026 with the review to extend/go permanent. Focused on serving those who matter most to us - our clients - by delivering compassionate, person-centred care in their homes.
Location: Salem Home Care, Toowoomba, QLD
Remuneration: $42.19 - $50.03 p/h (experience dependent) + Super + Salary Packaging + Staff discounts & benefits.
Role Type: Part Time (Mat Leave Contract) | Until March 2027 | 60hrs Fortnight
Hours/Shifts: Mon to Fri | Availability on weekends & public holidays as needed | Position may include travel outside of Toowoomba Region

- Deliver person-centred nursing care in clients’ homes.
- Conduct nursing assessments, develop nursing care plans, and complete clinical documentation.
- Provide clinical advice to clients and families, ensuring dignity, choice, and independence.
- Collaborate with Home Care Managers, Coordinators, and external health professionals.
- Ensure compliance within Aged Care Quality Standards, legislation, and Lutheran Services policies.
About You:
- Current AHPRA registration (Nursing).
- Strong clinical assessment and care planning skills.
- Ideally experienced in home care & aged care or community nursing.
- Current Full Australian Driver’s Licence and a fully comprehensively insured and reliable vehicle. Must be willing to drive between locations where required to do so.
- Ability to support care partners and support staff with clinical needs.
- Current National Police Check (or willing to obtain).
