We are seeking an experiencedRegistered Nurseto join ourCommunity Services Team, supporting older people to live safely and independently at home under theSupport at Home Program.
This role combinesclinical care, care coordination, education, and advocacy, working across home care services, group social support programs, and seniors activities. You will deliver person-centred care with a strong focus onwellness, reablement, and independence.
Key Responsibilities
Conduct comprehensive client assessments and develop care plans
Coordinate in-home services and liaise with external providers
Provide education, guidance, and advocacy for clients and families
Monitor care budgets and ensure compliance with aged care standards
Build strong, trusted relationships with clients and stakeholders
About You
CurrentAHPRA Registered Nurseregistration
Experience inaged care, community nursing, or care management
Sound knowledge of theSupport at Home Programand aged care standards
Strong communication, organisation, and time-management skills
CurrentNSW Drivers Licencewith a reliable, insured vehicle
Requirements

Valid AHPRA registration
