Community Worker – Full-Time
Total 247 Care Pty Ltd is seeking a permanent full-time Community Worker to provide community-based and in-home support services to clients in accordance with individualised care and support plans.
Key Responsibilities
- Provide direct support to clients in their homes and community settings
- Assist clients with daily living activities in accordance with individual service plans
- Support clients with community participation, recreational and social activities
- Assist with transportation to appointments and community activities
- Monitor client wellbeing and report concerns to management
- Maintain accurate client documentation, case notes and progress records
- Liaise with families, carers, health professionals and community service providers
- Assist in implementing and reviewing client support and behaviour programs
- Support implementation of care plans and promote client independence and social inclusion
- Ensure compliance with relevant NDIS standards, policies and procedures
Qualifications and Experience
- Certificate III or higher in Community Services, Individual Support, Disability Support, Ageing Support, Health Care Services, Caregiving or a related field
- At least one year of relevant experience in community services, disability support, aged care, home care, caregiving, health care support or a related field
- Strong communication and interpersonal skills
- Ability to provide person-centred support and work effectively with clients, families and multidisciplinary teams
